Please schedule your pharmacy transition appointment with a licensed Benefit Counselor. During your call, we'll review your eligible medications, explain the transition process, and coordinate the transfer of your prescription.

Most appointments take approximately 15–20 minutes.

During your appointment, we will:

• Coordinate your prescription transfer

• Answer your questions
